Write a 150-word email that:

  • States the issue in the first sentence (no preamble)

  • Explains the impact honestly

  • Describes what you’ve already done to fix it

  • States what you’re changing to prevent it happening again

  • Offers a call to discuss (with your direct number)

Tone: Transparent and accountable. No minimising, no blame-shifting, no corporate language.
